Welcome to PDS Health Information Task Force System (HIS)! This is accessible to PDS members in good standing and to residents in PDS accredited institutions.

PDS HIS Data Access Request Form 2024

  • Go to PDS website and click HIS Data Request tab
  • Fill in the required fields
  • For PDS members, select your name as the Supervising Consultant.
  • Download, sign, and upload the Data Use Agreement
  • Upload a Letter of Request addressed to the PDS President, through Research Publication and HIS Council Head cc HIS Chair
  • For year 2024, please address Letter of Request to:

  • Purpose of Data: HIS data may only be used for background information in grand rounds, case reports, research, and other academic purposes. Commercial and/or sponsored use will be subject to approval.
  • An email that contains a Data Request Access Link (accessible for 24 hours) will be sent to the specified email address
  • Click the link and fill in the information. If multiple terms are used, they should be enclosed in “ “ and separated by commas.
  • Specific age range and institutions may be selected (all institutions for central database, specific institutions for intra institutional requests)
  • The site will process the data request, and an excel file will be available for the requester to download.
  • Excel file contains 3 tabs:

Summary: Contains the actual disease counts: age, sex, new, new + follow ups, total

List of Diagnosis: Potential matches/matched diagnosis list

Search parameters: Data included in the search, search terms used
